Проблема была в отличии версий Extension Pack и самой VirtualBox.
В какой-то момент пару месяцев назад после очередного обновления машины, с включенным USB 2.0/3.0 начали крашиться. Выдает следующую ошибку

замечание vasek зачем корень закомментировали? И уберите указание по /dev
В fstab и загрузчике только по UUID или LABEL иначе рано или поздно проблем отгребете
Загрузился с livecd? проверил fsck'ом, все ок.

Было у меня такое когда создавал своп-файл, а не прописывал через swap.conf (динамический своп).
Поставил haveged - прошло.

pacman -S haveged
systemctl start haveged
systemctl enable haveged
Поставил, ничего не изменилось.
